**SPARTANS FUTSAL CLUB WAIVER AND RELEASE OF LIABILITY WITH VIDEO CONSENT**

I,(parent/guardian, above), hereby voluntarily allow my son(participant) to participate in activities organized and conducted by Spartans Futsal Club, including the X-Treme 3v3 League. In consideration for being permitted to participate in these activities, I, for myself, my heirs, personal representatives, or assigns, do hereby release, waive, discharge, and covenant not to sue Spartans Futsal Club, its officers, directors, employees, agents, and volunteers (hereinafter collectively referred to as "Spartans FC") from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or any of my property, while participating in any activity arranged, supervised, or sponsored by Spartans FC, including the X-Treme 3v3 League.

I understand that participation in futsal activities, including the X-Treme 3v3 League, involves inherent risks of injury, including the potential risk of exposure to communicable diseases, including but not limited to COVID-19. I voluntarily assume full responsibility for any risks and hazards involved in my participation in the X-Treme 3v3 League and hereby release Spartans FC from any and all liability for injuries or illnesses incurred during or as a result of my participation.

Furthermore, I grant permission for Spartans FC, its representatives, or agents to use any photographs, video recordings, or audio recordings of me taken during Spartans FC activities, including the X-Treme 3v3 League, for promotional, advertising, or educational purposes, in any medium, without payment or compensation to me. I understand that these images and recordings may be used by Spartans FC for various purposes and waive any rights to inspect or approve the finished product, including written or electronic copy, wherein my likeness or voice appears.

I understand and agree that my participation in Spartans FC activities, including the X-Treme 3v3 League, is entirely voluntary and that I have not been coerced or compelled to participate. I have carefully read this waiver and release of liability, and I fully understand its content and consequences. I am aware that by signing this document, I am releasing Spartans FC from any legal liability or responsibility for any injury or harm I may suffer while participating in the X-Treme 3v3 League or any other Spartans FC activities.

I acknowledge that I am at least 18 years of age, and if I am signing this on behalf of a minor participant, I hereby certify that I am the parent or legal guardian of the participant and have the authority to sign on their behalf.

I have read and understood this document and agree to its terms.

**VIDEO CONSENT AND RELEASE**

I (parent/Legal Guardian, above) grant Spartans Futsal Club the right to photograph, videotape, and record my image and my son's (participant) likeness, voice, and performance during Spartans FC activities, including the X-Treme 3v3 League. I understand that these photographs, videos, and recordings may be used for promotional, advertising, or educational purposes by Spartans FC without payment or compensation to me.

I waive any rights to inspect or approve the finished product, including written or electronic copy, wherein my likeness or voice appears. I release Spartans FC, its representatives, or agents from any claims, demands, or causes of action arising from these photographs, videos, and recordings.

I acknowledge that I am at least 18 years of age, and if I am signing this on behalf of a minor participant, I hereby certify that I am the parent or legal guardian of the participant and have the authority to sign on their behalf.

I have read and understood this video consent and release and agree to its terms.
